The selected course of this paper concerns the illnesses that entail nursing care in community health centers. All enrolled nurses who wish to strengthen their knowledge and enhance their ability in the care of clients with ongoing illnesses in a community health care center setting should enroll in this course (Bloomfield et al., 2023). Some of the aspects of chronic illnesses in the course will include, assessment, diagnosis, therapy, and prevention among many others.

Course Description
The fact that the morbidity rate is high in many community health centers, registered nurses should be informed of practices and techniques that would enable them to provide the best treatment for chronic individuals.
Vision
In other words, the goal of the course is to prepare registered nurses for efficient, high-quality nursing care of members in the community with chronic illnesses seeking services at the health center.
Rationale
Most of the individuals from different parts of the world live with fatal diseases that is chronic illnesses. Diabetes, high blood pressure, and general heart diseases are some of the top killers in America, and these are diseases that contribute a huge percentage to medical charges (Cohen et al., 2021). In the management of diseases and to achieve patients’ satisfaction, nurses are very important. Indeed many nurses do not possess adequate experience that is needed in treating patients with chronic diseases. This course helps meet these knowledge deficiencies by equipping the nurse with the ability and knowledge necessary to manage patients with chronic diseases to optimal potential.

Program Description
This continuing education program is for the registered nurse who wants to update his /her competencies and knowledge in providing care to patients at the CHCs to those with chronic illnesses (Cohen et al., 2021). This course will take 12 weeks and each week the learner should be willing to take 30 hours of training.

Improving the quality of the patient’s health will utilize nursing care, as a method of dealing with chronic ailments, by the implemented nursing practice. These are technologies that the nurses will employ in the management of care being delivered to those patients suffering from chronic diseases (Flaherty, 2019). They recommend that all patients with chronic illness should always be evaluated and supervised to make sure that their diseases do not have the following repercussions. A comprehensive care plan makes it easy for the nurses to comprehend the chronic patient problems quickly. Hence through the identification of the concerns of chronic patients, the nurses will be able to work as a team and therefore be in a position to provide the best care (Omboni et al., 2020). This will improve their teamwork. The leadership potential of the nurses will also be under development. It is important to demonstrate an ethical and critical approach to solving problems concerning chronically ill and complicated patients.
Identification of Assumption
However, the proposal is also based on the belief that registered nurses are willing to expand their experience on how to handle patients with chronic diseases in a community health center context (Van et al., 2023). They are encountered by numerous patients and are among the most important issues in many areas of healthcare. I believe that nurses will stand to gain from the many learning activities incorporated in the course, which will enhance their efficiency.

The current letter is to request approval to offer a Continuing Education course for Registered Nurses practicing in the community health center. The new proposed course is aptly titled ‘Chronic Disease Management in the Community Health Center Setting’ which would enable imparting the necessary knowledge and skills needed by the nurses in the management of patients having chronic diseases (Vo, 2021).
Chronic illnesses including diabetes, hypertension, and cardiovascular diseases are prevalent and nurses need to be prepared to respond to these patients effectively (Vo, 2021). The need for such a course will therefore be met as this course will enhance the knowledge of nurses on the pathophysiology, diagnosis, treatment, and management of Chronic diseases, as well as impart to the nurses the skills that will enable them to provide the requisite, optimum nursing care to patients with these diseases.
This course may be taken by nurses who may feel that they require enhancement in their knowledge level, to meet the standards of care expected of a health center that mainly deals with patients with chronic diseases within the health center (Haahr et al., 2020). Pre and post-course knowledge assessment checklists, skills practice, and post-appraisal will be used in the assessment of the effectiveness of the course. Likewise, skills can be assessed through detailed demonstration and tests, questionnaires that can also be used to determine the extent to which, a course improves productivity among the nurses involved (Fitzgerald, 2020). Employing the assessment design method, an assessment of the efficiency of the course in enhancing the knowledge and know-how of the nurses will be carried out.
As part of the evaluation design method, skill demonstrations, as well as assessments, still leave one guessing how real the above-said simulations would be in portraying a real-life patient care environment in a CHC (Fitzgerald, 2020). Perhaps, more studies could be done to identify the best ways of evaluating the impact of continuing education programs on patients. Literature supports the need for the suggested course. This training will enhance our program offering and enable the nurses to acquire vital knowledge and skills in the management of chronic illnesses.
